There was a major renovation in 1963, and the hospital closed in July, 1999, when the new SullivanCounty Community Hospital was opened. The 1963 addition is till standing and is being used by Dr. Custodio Lim.Witham Memorial Hospital%u2014%u2014Lebanon, Indiana%u2014%u2014Established 1917Founded with a $15,000 grant from Flavious J. Witham, a Boone County farmer, it served until 2003,when a 128,000-square-foot replacement hospital was constructed, called Witham Health Services.Hospitals
