A company representative said she believed it was created by a Mystic customer, but could not identify the individual, other than by the facsimile signature on the bottom of the sheet%u2014Joseph, whose last name is illegible. His name likely begins with an %u201cI%u201d to correspond with the serial number%u2019s suffix %u201cJI%u201d.
